How to make a REFERRAL
Our referral form gives us the information we need to make sure we can support the young person in the right way. If you need help filling it in, you can call us Monday - Friday from 9.30am to 5pm on 0161 228 7321.
The information below will make it easier, so have a look and then please complete the referral form as fully as you can. All young people referred to 42nd Street need to have given their consent, and you’ll be asked this on the form, so make sure you’ve discussed this with them

What happens next?
After our Duty Team have had a good look at the referral form to make sure we’re able to provide the right kinds of service that the young person needs, one of our Practitioners will be in touch with the young person to arrange a suitable time to talk on the phone so that we can assess their needs more fully.
This is called a Service Assessment.
If you’ve given us permission to, we may give you a call as well to fill in any gaps in our knowledge about the young person you’re referring.
