Healthcare Transition Workshops

We want to learn about your experiences and views on moving from child to adult healthcare services. 

This could include the transition from Child and Adolescent Mental Health Services (CAMHS) to an adult’s Community Mental Health Team (CMHT) or moving from a paediatric to an adult’s consultant for a physical health condition. Your insights will help us understand what matters most to young people, families, and carers during this transition, and will shape future healthcare planning in Greater Manchester.
 


What Do We Mean by Transition?

Transition is the process of moving from children to adults' healthcare services. This includes the full journey, from the initial planning steps to your transfer between services, alongside the support you receive throughout. Transfer is the actual point at which the responsibility for your care moves from a child to an adults' provider.


Who Can Take Part? 

The workshops are open to young people aged 16-25, family members, and carers, who have experience of moving or are preparing to move from child to adult healthcare services. We welcome your voices, whether you are a young person, parent, carer, or sibling. 

Taking part is completely voluntary and confidential. You can choose whether or not to join, and you are free to withdraw at any point during the discussion; you don’t need to give us a reason for this. There is no obligation to answer all questions, and you can participate in a way that suits you best.

What Will Happen in the Workshop? 

  • Format: The discussion will take place in person or online, depending on what works best for you and the group. We will let you know which format it will be before you confirm your participation.  
  • Duration: The session will last about one hour to an hour and a half.  You’ll be able to take a break if you need at any time.  
  • Discussion: We’ll have an open discussion about your perspectives and experiences of healthcare transitions, loosely structured around a couple of activities which we’ll share with you in advance. This will be a safe and supportive space where we will welcome anything you have to say. You might want to share some negative feedback, but please be reassured that this won’t affect how any healthcare workers think of you; we’re keen to know all we can to improve your experience of services!  

Will I Be Reimbursed? 

If you need to travel to attend an in-person workshop, we can reimburse your transport costs. Once we’ve confirmed which group you’ll be attending, we will share some information on how to ask for this.  

We will also provide you with a £20 voucher to thank you for taking the time to share your thoughts.  

How will we use the information we collect? 

The ideas, experiences, and suggestions you share will help us create a report that reflects what matters most to young people and families during healthcare transitions. This report will be written up by the 42nd Street team. 

Your contributions will always be anonymised in the report, so that others aren’t able to tell who said what, and they will only be used for the purposes of this project 

The final report will be shared with NHS England and other healthcare organisations across Greater Manchester. It will also be given to relevant members of the Strategic Clinical Network who are able to make changes within the organisation based on your feedback. Your input will help inform decisions and plans for future health services, aiming to make transitions smoother and more supportive for young people and families.
